The full patient history should be included along with all diagnostic results relevant to the diagnosis. Kindly ensure your team is aware that I may request additional information. You will be sent a copy of the clinical meal plan for referred patients. The meal plan is normally ready within 1 week of the clinical nutrition appointment.
